RheoMed understands that independent practices want to remain autonomous. We are private practitioners working collectively to leverage our resources to provide excellent care by promoting care management.

RheoMed is different because of its leadership. At every level of the organization, the board, the committees, the quality improvement, it 's all nephrology led and nephrology owned.

RheoMed is doctor-controlled and doctor-run. We as physicians are in practice, taking care of patients, and identifying and addressing challenges that are a part of our patient's care, every day.

We offer transitional care visits to help patients. Once they've been discharged, we ensure that they’re safe in the outpatient environment, reconcile their medications, and assisted them with any necessary care continuity. These are things that, in the past, haven’t been options. For a small practice, the physician doesn't have the time, manpower, or financial ability to do any of this.
